The A4LI Foundation Patient Access Program is a free navigation service for patients with serious age-related illness who have exhausted approved treatment options and need help connecting to licensed treatment facilities.

What we do.
The longevity therapeutics landscape is moving rapidly — but for patients facing serious age-related illness, navigating it is extraordinarily difficult. Experimental treatments are increasingly available through licensed facilities, but finding trustworthy information, understanding eligibility, and connecting to the right providers requires expertise most patients and families don’t have.
The Patient Access Program connects individuals over 50 and families to vetted, licensed treatment facilities and trustworthy clinical information — at no cost. We do not recommend specific treatments or provide medical advice. We connect people to institutions that can.
Who this program is for
Adults with a serious age-related diagnosis (cancer, neurodegeneration, cardiovascular disease, metabolic disease, or advanced aging-related frailty) who have exhausted or are ineligible for approved treatment options and are seeking information about experimental or emerging therapies.
Important notice
The A4LI Foundation is not a medical provider. We do not diagnose, prescribe, or treat. All clinical decisions must be made in consultation with licensed physicians. Our service connects patients to licensed facilities — it does not replace medical care.
How it works

Submit an inquiry
Complete a brief intake form describing your diagnosis, treatment history, and what you’re looking for. All submissions are confidential.

Our program staff — including advisors with clinical and scientific expertise — review your inquiry to understand your situation and identify relevant options.

Personalized navigation
We provide you with a personalized summary of licensed treatment centers, relevant clinical trials, and vetted information sources matched to your situation.

Ongoing support
We remain available for follow-up as your situation evolves. We do not disappear after one referral.

Licensed & vetted
We only refer patients to facilities operating under applicable state and federal law. We do not connect patients to unlicensed providers.
No cost, ever
The Patient Access Program is funded entirely by philanthropic support. Patients are never charged for navigation services.
Confidential & Private
Your medical history and personal information are protected throughout the process. We will never share patient details with any outside party without your explicit consent.
